I consent to Carver’s use of photographs or likeness of myself and anyone assigned to me or my child(ren) for use in promotional materials or team films.*

I release Carver Community Center, its board, staff or volunteers from liability in event of accident or injury for myself; or as a parent/guardian to my child(ren).*

I consent for my child(ren) to participate in Carver programs.

I give my permission for child to use the internet.

I understand that I am required to provide my camper with lunch each day that they attend camp.
